Pharmacy Corporations and Chains
Corporate offices should receive a list of all the stores that your corporation owns. If the Board has underestimated the number of stores owned by you corporation, please send additional renewal fees to reflect the accurate number. Please note that applications will not be processed unless renewal fees for all the stores within the chain are received by your corporation. Payments may be made in person at the Board office with Visa or MasterCard only.

Important Sterile Compounding Information
The Sterile Compounding Permit Program was repealed during the 2015 Legislative Session by the passage of SB 69/HB 181 entitled “State Board of Pharmacy – Sterile Compounding – Compliance by Nonresident Pharmacies and Repeal of Permit Requirement,” effective April 14, 2015. Therefore, there is no separate permit required for a pharmacy performing sterile compounding pursuant to a patient specific prescription.
