How can I obtain a duplicate license?

Mail us a request in writing with appropriate fee to the corresponding P.O. Box address you can find here: http://dhmh.maryland.gov/pharmacy/Pages/contact-us.aspx

The fees are as follows:

• $30 for Pharmacists
• $10 for Pharmacy Interns and Pharmacy Technicians
• $25 for Written Verification of Good Standing
